The database includes pay information provided by the individual entities to the State of Utah, or provided to UtahsRight.com under a Government Records Access and Management Act request, or GRAMA. In those three years, over 900 licensed professional have been sanctioned from over 475 different companies. Nearly 75 different professions and company types are reflected in the numbers.

DOPL licenses and regulates many profession throughout the state, including medical, trade and construction professions. The highest number of violators in the three-year period came from the construction trade, with 556 violators.

The second highest profession to be sanctioned were physicians and surgeons, who had 61 violators, followed by cosmetologists/barbers with 55 violators and massage therapists, who had 50 violators. Other highly sanctioned professions include unarmed private security officers, who had 36 violators, and dentists and hygienists, who had 35 violators.

The database, which holds over 800 records, lists liquor law violations for the last 10 years (June 2001- June 2011), listed by establishment name. Addresses and locations are listed with establishments when available.

Establishments were fined or had their liquor licenses temporarily suspended for many different offenses, most often for serving alcohol to minors, employees not having a name badge, or allowing non-members to enter the bars (for earlier years when memberships were required to enter bars).

Only licensed establishments that received violations from the DABC are included in this database. Violations that were later dismissed are not included. If a citation is listed as still being adjudicated, but the business no longer has a liquor license, the alleged violations are listed in this database.

The violations that occurred most often within the facilities centered mostly on lack of paperwork, with 65 violations of failing to report a new individuals information, and 61 offenders dinged for failing to follow all rules and regulations – most often dealing with having proper immunization records.
The third highest violation was having sharp objects, edges, or points accessible to children, with 55 violations in January through August 2011.

The Salt Lake Valley cities included in the numbers were as far south as Draper and as far north as South Weber.

The data was compiled by UtahsRight.com for a weekly series in The Salt Lake Tribune’s neighborhood section highlighting information gleaned from public databases. The information was received from a public records request to the Utah Department of Health’s Child Care Licensing.

Though Salt Lake City had the highest number of violations in the Salt Lake Valley area, with 99 violations, they also had the highest number of facilities, with 113 operating in the city.

Kearns and South Weber had the highest average number of violations per facility, both with an average of 1.3 violations. South Weber had four violations in their three licensed facilities, while Kearns had 16 violations in the 12 operating childcare facilities.

Several cities had zero violations in the examined time frame, including South Salt Lake, Centerville, Sunset and Syracuse.

Along with salary information, the divorce database and our criminal charges database have both been updated to reflect more current data. 

The 84401 ZIP code, which includes parts of Ogden, Hooper, Marriott-Slaterville, Roy and West Haven, had 39 burglary probationers and parolees residing in the area during the third week of January 2011, according to data from the Utah Department of Corrections.

 The Salt Lake City and West Valley City area within the 84120 ZIP code had the second highest number of probationers and parolees residing in the area, with 21 offenders. Behind them, the 84116 ZIP code, which includes part of Salt Lake City, had 19 offenders residing in the area during that time frame.

The convicted offenses included all types of burglary crimes, including burglary of a dwelling, a building or a vehicle. The number of probationers and parolees does not include those who have absconded or are in a hospital or in a correctional facility for another offense.

One ZIP code — 84119 — had no probationers or parolees convicted of burglary crimes registered as residing in the area. That ZIP code includes parts of Salt Lake City, South Salt Lake and West Valley City. The areas in ZIP codes 84092, 84014 and 84098 had only one offender living in their area. Those ZIP codes include the areas of Sandy, Alta, Snowbird, Centerville and Park City.

The childcare database includes 766 new citations from providers throughout the state. Of those citations, 47 were due to choking hazards being present for children under the age of 2, 29 had toxic/hazardous materials within reach of children, and 18 had unlocked firearms in their home.

The childcare database can be browsed in its entirety, or can be searched by provider name.

Forty-one new entries were added to the real estate database, including a mortgage loan originator who used personal funds as a down payment on a loan for another person, and a residential appraiser who failed to report and analyze relevant sales and listings when making an estimate.

Over 1,500 real estate records are available online, spanning the last seven years. The database can be browsed in its entirety, or can be searched by provider name.

The criminal charges database contains over 800,000 records dating as far back as 1997, and includes all district court cases, except those that have been dismissed or expunged.

The divorce database contains over 150,000 records, and includes statewide divorces since 1997.
